Director Andy Sidaris intended to shoot this movie exclusively on IMAX film. After filming several scenes, an attempt to mount the very heavy and expensive IMAX camera on a Cessna to film aerial footage resulted in the plane overshooting the runway on takeoff. The weight was far beyond what the single engine Cessna could handle. The pilot required several surgeries before he could return to some semblance of a normal life. Andy Sidaris was so angered by the expensive loss of equipment that he stipulated in his last will and testimony that the pilot's name be removed from the credits for the Blu-Ray release.
Ava Cadell's first appearance in the series (as a hit woman). She appears in the subsequent films as the DJ/sexologist for KSXY Radio.
The only post-1985 "L.E.T.H.A.L. Ladies"-series film directed by Andy Sidaris or Christian Drew Sidaris that does not feature actor Rodrigo Obregón in any role.
