In Tyler's apartment, when Tyler offers Lipton coffee, the pot is brewing and is over half full. In next shot, when Tyler goes to pour the coffee, the pot is still brewing but is only about 1/8th full.
[43:00]When McCarthy is in bed and gets the call regarding the murder of Ellen at Tyler's apartment, he reaches for the phone and knocks a bowl of peanut shells on the floor that are next to the phone, when he is finished on the phone he hangs it up, and the bowl of peanut shells is back on the table.
(at around 13 mins) Rollie Tyler leaves the FBI office, then in the next shot he is walking up the stairs to his apartment wearing a black sweater that he didn't have on in the previous shot.
(at around 36 mins) When Rollie bundles Ellen into her apartment block they leave the key in the front door.
During the fight scene in the kitchen an orange flame from the stove appears when a teapot is removed. Any stove flame is blue or oxygenated while this is orange or reducing. Therefore this was done for visual effect and is not the correct flame that a stove produces.
Rollie Tyler is on a payphone talking to Mason who asks him where he is. Tyler looks around looking at street signs to determine where. Payphones had the exact address on a label on them.
Varrick (Tom Noonan) carries a S&W Model 19 snub-nosed revolver, but his holster is clearly for a longer barrel gun.
Colonel Mason tells Rollie that there's $15 million of mob money in a safe deposit box at a bank in Geneva, which he and Leo go to pick up, with Rollie disguised as DeFranco "because only DeFranco & I are known in Geneva," as Mason had told him. This box turns out to be hardly larger than the gym bag Rollie takes out of it. Given that in 2017 $20 million in fraud-gotten cash was discovered taking up 95% of the space inside a box spring, neither of these would've been able to contain all of that money.
The Justice department doesn't issue social security numbers. The Social Security Administration does that.
[41:00] When Rollie moves Ellen from the floor to the bed after she has been shot, there is no blood or stain on the floor. Her injury was fatal, and though the movies have conditioned us to think there should be lots of blood from a fatal gunshot wound, a person who is shot in the thorax or abdomen can die from internal injury/bleeding with relatively little external blood loss thus there may not have been a large amount of blood on the floor.
William J. Adams was supposedly born in Massachussetts, but his social security number prefix is 546- which at the time would have meant it was issued in California. However, the SSN may not have been issued where he was born, he may have relocated to California as a child then was issued his social security number later. Although it's common in the 21st century to issue a social security number to newborn babies, in the 1980's some people didn't get one until they were an older child (eg. junior high school or high school age).
[42:00]As Rolly is fighting with Ellen's killer, the rifle he is beating him with bends as he hits him, showing it is a 'rubber ducky' prop.
The uniforms worn by the New York State Police at Mason's mansion and the morgue are their dress blouses and are only worn for special ceremonies such as funerals, parades and awards and are never worn on patrol.
In the scene when Roland is beating the assassin's back with the machine gun you can see the butt of the gun wobble, as it is made of rubber.
When the F/X van is being followed by the Lipton, the light on Lipton's car is on the driver's side. In the rear-view mirror shot, the light is on the passenger side.
When the movie goes into closing credits, after the first few they show quick shots of the main players in a way you'd expect to see the actor's names credited at the bottom (Like a John Landis movie). But oddly, there are no names seen.
The name of the fictional "Royal Bank of Geneva" makes no sense, centuries have passed since Geneva was last ruled by any monarchy.

At 1:18:55 during the chase scene as the F/X van is being stolen from the impound lot, crew can be seen with a camera as cars start to collide. Also, the F/X van that is supposed to be pursued can be seen parked behind the crew.
When Rollie pushes Ellen into the apartment after the shooting, you can see the keys still dangling in the lock outside.
After his attempted killing in the phone booth, Rollie goes home for the night. Even an idiot would know that's the first place they'd come looking for him. (And they did.)
At the end of the movie, Rollie and Leo go to Geneva to get the money. By then the coroner's office fake out would have been noticed and Rollie would have been wanted for questioning in several murders, and Leo would have been wanted for stealing his boss's badge and impersonating him. Even if Leo had arranged fake passports for them, they would have been spotted at the airport.
[29:00]When Rollie shoots DiFranco, he is later uncertain whether the revolver was loaded with bullets or blanks. An experienced special effects professional would immediately notice the difference in noise and lack of recoil caused by blanks.
