There is a scene where you get a flashback about a rich guy and how he wasted his money and you can see him putting his face into the cleavage of a hooker.
During one mission, the player must explore a brothel. Several sexually suggestive conversations from splicers are overheard though nothing directly referenced or shown.

Other references to dictators cult leaders throughout the city's history and their violent scare tactics to keep citizens in line and loyal to them.
Players should also note that there several references to religion and the occult (the citizens worship Sofia Lamb as a messenger of God and call her daughter The Lamb of God and Leader of The People and pray to her in a religious fashion and spread her name and Sofia's teachings as gospel).
One reference to breaking a woman's jaw and her needing surgery to heal it.

The main theme of the game is that the citizens of Rapture all use "Plasmids" which are drugs (injected into the skin) to give the user super powers.
Multiple times through the game the player is required to "splice" the drugs to make themselves more powerful.
The player can drink until they get drunk and the screen gets blurry and audio gets distorted. If you smoke the health bar drops.

A lot scarier the first game, frequent jump scares, and a very disturbing atmosphere. The is by far the scariest game in the series and should not be played by kids, and teens who play this need parent supervision.
