The client's son reported that the client was having difficulty urinating and defecating after surgery. On assessment, the client showed signs of irritability, restlessness, guarding behavior, and had a Foley catheter in place. The nursing diagnosis was impaired urinary elimination related to the post-surgical procedure. The short-term goal was for the client to become independent in elimination within 5 minutes of nursing care. Plans included providing information to the client, checking the catheter, and assessing factors impairing elimination.
