PARENT`S CONSENT

I / We, Mr. Longino L. Canillo and Mrs. Nila G. Canillo parents/guardian of Dave G. Canillo, a
prospective Student-Trainee of Cebu Technological University Main Campus, have hereunto grant permission
for my son/daughter to undergo 400 hours On-the-Job Training (OJT) in Austal Phil. Pty Ltd.
Arpili Balamban, Cebu which is a requirement for the completion of the course, Academic Year 2014-2015,
2nd Semester.
That we made it known our continued financial and moral support to our son/daughter during the training.
That we shall adhere to any disciplinary action of the school, such as dropping him/her from the rolls of trainees
and/or barring him/her from graduation should it be found that he/she is a frequent absentee and/or notoriously
undesirable trainee.

precautionary and preventive measures are being implemented to prevent any accident to happen.
