30. Steps or preparation for confine space?
Ans.
Equipment Shutdown
Isolation (Lock, Tag, Clear & Try)

SINGLE BLOCK VALVE

An isolation method for routine maintenance activities and which is least desirable process
for use in hydrocarbon service. (e.g. Changing a gauge or sock filter, temporary isolation
while a blind is being installed or cleaning of inline strainers).

DOUBLE BLOCK AND BLEED (DBB)

An isolation method for locking and tagging two consecutive valves on the same line with a
drain valve installed and open for bleeding. (e.g. Removing a control valve).

DISCONNECTION / PHYSICAL ISOLATION OR SEPARATION / REMOVAL

A less frequent isolation method for long term maintenance activities by physically removing
a piece of equipment from the system. (e.g. Remove a spool from a pipeline).

BLINDING / BLANKING
An absolute closure of a pipeline or duct, by fastening across its bore with a solid plate or
cap which extends to the outer edge of the flange at which its attached and capable of
withstanding the maximum upstream pressure. (e.g. Blind Flange, Slip Blind, Spectacle Blind,
Stand-off Blind, Plumbers Plug & Hammer Blind).

PHYSICAL RESTRAINT / CHAIN VALVES

An isolation method by physically restraining, blocking or securing the movement of the
equipment. (e.g. Disconnect belt, and chain drives and mechanical linkages on shaft-driven
equipment, securing mechanical moving parts within a confined space with latches, chains,
chocks, blocks or other devices. Pin a fly wheel from moving; secure a fan blade from
moving).

ELECTRICAL ISOLATION (Qualified Electricians)

An isolation method to de-energised or physically turn off and disconnect machinery or
equipment from the energy source by a competent and qualified personnel. (e.g. Open
breakers, Remove fuses & Disconnect wiring).

LOCKOUT & TAGOUT (LOTO)

A method to safe guards the employees from the unexpected energization or startup of
machinery and equipment. (e.g. Install approved and identifiable lock and tag by every
individual working on the equipment).
De-pressurize / De-energize
Decontamination
Draining
Flushing
Cleaning
Purging (Water Washing, Steaming, Nitrogen)
Ventilation (Ventury, Exhaust Fan, Ventilator / Air Mover)
Gas Test
Maintenance Activities
