thereby has not been sold, transferred, assigned, or pledged to any person whatever and that Affiant is entitled to the full and exclusive possession of said item.

This affidavit is made for the purpose of inducing JPMorgan Chase Bank, N.A. (“the Bank”) to issue to Affiant a replacement item or to redeem and pay

to Affiant the proceeds of said item or the amount due thereon by reason of Affiant’s ownership thereof. Affiant understands that the Bank may require that a

surety bond be posted before it replaces or redeems the item.

In consideration whereof, Affiant agrees, at all times hereafter, to save, defend, hold harmless and indemnify the Bank, its successors and assigns, from

and against said item and all claims of whatever nature arising therefrom and from all costs and expenses, including, but not limited to, attorney fees to the

extent allowed by law, that may arise from the presentation, collection, or attempted collection of such original item.
