Sangam Certificate or Community Certificate on Original. 5. Ration Card or Electoral Card, Xerox copy attested by the Gazeted Officer. 6. Affidavit on Rs.10/-worth Non-Judicial Stamp paper with Notarized. 7. Enclose the same certificate, if previously taken. 8. Affix one recent passport size photograph.
