The 4th Edition International Standards for Hospitals (Effective 1 January 2011) require organizations to select 5 clinical measures from the JCI International Library of Measures (see QPS.3.1). A list of the measures from which to select the five will be posted August 16th on the JCI website. On August 25th the full measure set will be posted on the Resource Center for Accredited Organizations. This posting will contain the full description of the measure with data definitions and all associated collection criteria. To give readers an advanced look at the measures they are provided below. Acute Myocardial Infarction (AMI) Aspirin received within 24 hours of arrival to the hospital for I-AMI-1 patients having an acute myocardial infarction (AMI). Aspirin prescribed at discharge for patients who had an acute I-AMI-2 myocardial infarction. ACEI (angiotensin converting enzyme inhibitor) or ARB (angiotensin receptor blocker) for patients who have LVSD (Left Ventricular Systolic Dysfunction) after having an acute I-AMI-3 myocardial infarction. Adult smoking cessation advice/counseling given to patients who I-AMI-4 had an acute myocardial infarction. Beta-blocker prescribed at discharge for patients who had an acute I-AMI-5 myocardial infarction. Acute myocardial infarction (AMI) patients who expire during the I-AMI-9 hospital stay Heart Failure (HF) Heart failure patients with documentation in the hospital record that left ventricular systolic (LVS) function was evaluated before I-HF-2 arrival, during hospitalization, or is planned for after discharge ACEI (angiotensin converting enzyme inhibitor) or ARB (angiotensin receptor blocker) for heart failure patients who have I-HF-3 LVSD (Left Ventricular Systolic Dysfunction)

Measure Description
Adult smoking cessation advice/counseling given to heart failure patients
Stroke (STK) Patients with ischemic stroke prescribed antithrombotic therapy at I-STK-2 discharge Patients with atrial fibrillation/flutter receiving anticoagulation I-STK-3 therapy Stroke patients who were given stroke education during their I-STK-8 hospital stay Ischemic or hemorrhagic stroke patients who were assessed for I-STK-10 rehabilitation services Childrens Asthma Care (CAC) Pediatric asthma patients who received relievers during this I-CAC-1 hospitalization Pediatric asthma patients who received systemic corticosteroids I-CAC-2 during hospitalization Hospital-Based Inpatient Psychiatric Service (HBIPS) Psychiatric patients who were placed in physical restraints during their inpatient hospitalization. This measure will determine the total number of hours that patients were maintained in physical restraints for those admitted to a hospital-based inpatient I-HBIPS-2 psychiatric setting Psychiatric patients who were placed in seclusion during their inpatient hospitalization. This measure will determine the total number of hours that all patients were maintained in seclusion for those admitted to a hospital-based inpatient psychiatric setting.

Nursing-Sensitive Care (NSC) Patients that have hospital-acquired (nosocomial) pressure ulcer(s) (category/stage II) on the day of the prevalence study.<BR>Note: I-NSC-2 Please see Appendix E for details on how to collect this measure. All documented falls with or without injury, experienced by I-NSC-4 patients in a calendar month. All documented falls by a patient with an injury level of minor (2) I-NSC-5 or greater.
