4.5) Brief description of the incident:---There was fire in the tarpaulin (used for making welder shed) over the nose block assembly. It occurs due to the fall of spatter while gas cutting from the pressure part panel (aprox elevation +72 mtr). The persons inside the tarpaulin came out and tried to extinguish with the help of fire extinguisher. Once the persons applied the fire extinguisher it got controlled but it was not extinguished fully as the gusty air facilitated the fire to spread Then the person dropped the tarpaulin on the ground. . Finally the fire was extinguished by fire extinguisher and the water tankers with the co-operation of timely approach of IR, execution and dedicated workers.

5. Causes of the Incident


5.1) Direct Causes: Conflicting activities in same line at different elevation. Usage of sub standard fire proof tarpaulin.

5.2) Root Causes (attach revised HIRA after review of probability, severity and control measures ): Lack of supervision Non deployment of helper (fire watcher) who will keep close eye on the hot work operation and inform the concerned persons regarding generation of unwanted fire. Multi tier working.

6. Precautionary Measures
6.1) What are the precautions taken / being taken to prevent similar occurrence?

Conflicting activities should be avoided Purchasing of sub standard fire tarpaulin to be stopped. The Precautionary measures that can arrest the spark and hot spatter at the source should be immediately implemented. Fire fighting technique to be communicated to the workers Training to be provided to all regarding evacuation technique at the time of emergency. Fire tender to be kept at the plant premises (preferably by client).
