Nursing Care Plan

The patient reported abdominal pain rated at 7 out of 10 on the pain scale, with facial grimacing and pale skin. Vital signs showed a temperature of 36.2 C, pulse of 96, respiratory rate of 94, and blood pressure of 120/80. The nursing diagnosis was alterations in comfort due to pain from disease. The plan was for the patient to decrease their pain level to 5 out of 10 within 8 hours through determining pain characteristics, positioning comfortably, using hot compresses and relaxation techniques like music therapy. After 8 hours the patient's pain had decreased and was rated at 5 out of 10.
