INSTRUCTIONS FOR UNDERGRADUATE

ADMISSION FORM-2009

Please read the instructions carefully before filling the Admission Form. Please type or
write legibly in BLOCK letters, and fill in all the fields. Incomplete form will not be entertained.
The instructions are numbered in accordance with the Admission Form.

1. Fill in the name in full exactly as on your SSC/O-level certificate.

2. Fill in your Father’s name in full.
3. Fill in the name of your guardian even if it is the same as 2 above. Also mention your
exact relation with the guardian.
4. Write complete contact information including postal address, residential telephone and
mobile number. E-mail address may also be provided, if available. This information is very
critical as it will be used for correspondence by the Institute during the entire admission
process.
5. Write permanent address as per father’s/guardian’s CNIC (Computerized
National Identity Card), including residential telephone and mobile
number.

6. Provide information about Father’s/Guardian’s occupation along with designation

and complete business/office/work place address.

7. Date of Birth of applicant (Same as per your official documents).

8. Write place of Birth, if born in Pakistan, write City and District of Birth. If born
Abroad, write City and Country of brith.

9. Nationality of the applicant.

10. In case of Pakistani applicants, write Province of Domicile. Non-Pakistani write NA.

11. In case of Pakistani applicant, write CNIC number or Form B number. Non Pakistani must
write Passport Number.

12. Total annual family income from all sources (Salary, Pension, Business
Investment Including Stocks/Bonds/NIT/IC P etc. , Property, and Agriculture).
13. Fill in the Academic Records.

§ Please delete the items not applicable to you in the certificate column.

§ If you have already completed the said certificate, write the year of passing. If you are
appearing in the 2nd part of HSSC/A-Levesl/Equivalent this year, please write 2009 in the
year column.

14. Indicate the preferences of faculty after careful consideration. Write a number from 1 to 6 in the
box next to the faculty name. DO NOT use ([) mark. Please note that by filling all the
available preferences, your chance of securing admission increases.

15. Please write the name of the Test Center where you would like to appear in the Admission Test.
Admission Test will be held simultaneously at seven Test Centers.
16. Payment Instructions:

a) Local applicants please attach receipt of payment of Rs. 800/- (Rs. 1500/- in case of
using downloaded form) of HBL online payment to A/C No. 19790000085901.

b) Overseas applicants please attach receipt of payment of US $ 65/- (US $ 135/- in case of
using downloaded form) of HBL online payment to A/C No. 19790002044611.
Also write the Online Receipt No. in space provided.

- Admit Card: Please fill the correct address on the Admit Card clearly. Paste one
recent Passport size photograph in the space provided.

- Attested copies of Mark Sheets and Certificates : You are required to enclose attested copies
of SSC/HSSC or O-Levels/A-Levels/Equivalent along with the Admission Form. Attestation can be
done by a Gazetted Officer or the Principal of the school/college last attended.

- Hope/Surety Certificate: If you are appearing in the 2nd part of HSSC/A-Levels/Equivalent this
year, you are required to submit Hope/Surety Certificate issued by the Principal of your
College, stating that you will secure the requisite marks/grades.

- SOPREST Financial Assistance Form: Financial Assistance Form may be purchased from
HBL Designated branches on payment of Rs. 500/-. You are required to read its Instructions
carefully and provide copies of all relevant documents to facilitate processing.

- Provincial Scholarship Form: The Scholarship Form can be downloaded from the web site of
GIK Institute www.giki.edu.pk. The complete form and relevant documents must be submitted along
with Admission Form. Please note that the Institute may verify particulars of an applicant
independently. The parents/guardian of the applicant may be called for interview.

17 Certificate/Undertaking: An Undertaking Certificate is given at the end of the Form.You are

required to read it carefully before signing. It must also be endorsed by your Father/Guardian.

Please bring along this card to the Test Centre. You may not be permitted to
appear in the test without this card.

Please reach the Test Centre at 9:00 a .m.

Clip board and ball-points will be provided at the Test Centre.
Do not bring your own writing material. Calculators, mobile phones or any helping
material are not allowed at the Test Centre.
Use of unfair means will lead to immediate expulsion from the Test Centre and
cancellation of the admission test.
