Agency Instructions
Please complete one questionnaire per data center. A data center is defined as an agency space containing 10 or more PHYSICAL OR LOGICAL servers, any of which supports a strategic or nonstrategic information technology service. Please save each completed questionnaire (Excel Workbook) in the following electronic format: "DCC Response - Agency Name - Data Center Name.xls". Please complete your data center questionnaire(s) and return in electronic format to: AEIT_DCC_Questionnaire@aeit.myflorida.com by May 8, 2009.

Purpose: The data center information collected in this questionnaire will assist the Agency for Enterprise Information Technology (AEIT) with identifying, by October 1 of each year, at least two non-primary data centers for consolidation into a primary data center. Primary Data Centers are those identified in Florida Statute, Ch. 282.0041. This information will also assist AEIT with developing and submitting to the Legislature an overall consolidation plan for state data centers by December 31, 2010. A second, more detailed, inventory questionnaire will be sent to those agencies identified for data center consolidation. The information collected in the second questionnaire will facilitate consolidation and migration planning.

Analysis Methodology: AEIT will review the information collected in this questionnaire to assess costs, risks, constraints, and requirements for consolidating agency data centers into primary data centers. Specific areas of analysis include: agency data center financial (funding) information; agency data center facility (location, condition, and square footage) information; agency data center equipment (refresh and virtualization status) information; and agency data center personnel (administration, maintenance, and failover/recovery) information.
