RELEASE AND QUITCLAIM

KNOW ALL MEN BY THESE PRESENTS:


That I, ______________________________, Filipino, of legal age, a resident of________________________
________________________________________ and formerly employed with Manpower Outsourcing Services,
Incorporated,
do
by
these
presents
acknowledge
the
receipt
of
the
sum
of
________________________________________________ as full payment of all wages, salaries, bonuses and
other benefits
In consideration of my receipt of the above sum:
1.

I confirm the cessation of my employment from the Company effective ________________

I acknowledge the value of the opportunity afforded me to be of service to the Company.

3.

I acknowledge that my last pay, remaining benefits and other compensation due me are as contained in the
final computation attached to this quitclaim. I hereby acknowledge that the same is a correct and accurate
computation of all the salaries, allowances, 13th month pay, bonuses, leave credits, sickness benefits, tax
refunds and other benefits due me under the law. I understand that this amount shall be credited to my payroll
account. Upon crediting of said amount to my payroll account, I hereby declare that I would have no more
claim or cause of action whatsoever against the Company, its clients, their respective stockholders, directors,
officers, agents, and employees in connection with my employment or cessation thereof.

By reason of the receipt of the said amount, I hereby release, remise and forever discharge the Company,
its client/s , their respective stockholders, directors, officers, agents, employees from any action, claim for
sum of money, or other obligations arising from all incidents of my previous employment with the Company
and/or assignment with its client/s .

5.

I acknowledge that I am physically fit for work and, therefore, am not suffering from any disability.

6.

I will not anytime, in any manner whatsoever, directly of indirectly, engage in any activity prejudicial to the
interest of the Company, its stockholders, directors, officers, agents and employees and will not disclose any
information concerning the business of the Company, its clients, or their manner of operation, plans and
finance.

7.

I acknowledge that I have no cause of action, complaint, case or grievances whatsoever against the
Company and its client/s , their respective stockholders, directors, officers, agents, and employees in
respect of any matter incident to or arising out of my previous employment. I further warrant that I will
institute no action against the Company, its client/s , their respective stockholders, directors, officers,
agents, and employees and will not continue to prosecute any pending action which I may have filed or
which may have been filed on my resignation shall not be taken by me, my heirs or assigns as a confession
or an admission of liability on the part of the Company, its client/s , their respective stockholders, directors,
officers, agents and employees for any matter, cause, demand, or claim for damages which I may have
against any or all of them.

8.

I finally declare that I have read and understood this document, and the release, waiver and quitclaim
hereby given are made by me willingly and voluntarily and with full knowledge of my rights under the law.
