Dear Sir,
We the undersigned propose to establish an NGO in accordance with the provision of the
voluntary Social Welfare Agencies (registration & Control) Ordinance 1961 (XLVI 1961). The
particulars of the proposed Agency are as below.
1. Name of NGO:

Rahman Welfare Foundation.

2. Address: Hujra Said Rahman Village Bakrai, P/O Badragga,Tehsile Dargai, District
Malakand.
3. Aims & Objects: As per Constitution of the Rahman welfare Foundation.
4. Area of operation: KPK
5. Plan of operation: Attached
6. How proposed to be financed: Local contribution, Donation, Membership fee, Grants-inAid

thirty days thereof.

Take measure to strengthen women participants in development activities.

To provide the means for people (male, female)to obtain basic Education.

To engage in any other development and capacity building activities that are conductive
to the uplift of the poorest neglected sectors of the society.

To undertake health and nutrition with special focus on the poor and malnourished
women and children.

To empower the marginalization segments of the society through their involvement in


decision making and participants in the project activities.

Progress report:
Health issues are emerging and health facilities are very scarce. Due to these issues and lack of
knowledge and health education life expectancy compare to developed countries is low as well mortality
ratio is also low. These issues needs the attentions of the community because when government fails to
fulfill basic needs of citizens, then citizens should take step to help state in resolving these issues at
community level. And when each community coordinates state in resolving these issues state develops
and when community do not cooperate state machinery decline day by day.
Therefore Rahman Welfare Foundation has successfully completed three activities of the first year in the
specified time planned. Rahman welfare foundation completes three activities for the first year including
free medical camps in two different villages and distributed free medicines in poor peoples and free
checkup were also conducted, in the free medical camps special focus were children and womens.
Three hundred and twenty five women and two hundred childrens free cheek up were carried out and
free medicines were handover to poor peoples.
One health hygiene awareness raising campaign were carried out by RWF in the support of local
community to change the behavior of the local community regarding health. Different sessions were
conducted in different places in villages such as HH level, community level, school and religious places.
Total thirty sessions were conducted which reached the health hygiene messages to nine hundred peoples.
