Career Counselor
Provides direct SFL-TAP (Soldier for Life Transition Assistance
Program, formerly ACAP) services to transitioning military personnel,
Army civilians, and their family members. Conduct automated and
manual pre-separation briefings, delivery of job assistance training in
group or individual settings, conduct of individual counseling
sessions, and other job assistance activities such as resume critiques
and mock interviews. Provide career development and job search
counseling to service members in individual and group settings,
decided and undecided individuals. Participate in employer
development activities including employer visitations, advisory
boards and on-post recruiting, and the development of job and
internship leads. Supports SFL-TAP Center marketing initiatives by
briefing installation leaders and managers. Tracks the progress of

clients and motivates clients to increase their utilization of SFL-TAP


Center services. Manages supplies and assume responsibility for a
major activity such as job fairs, employer days, or the conduct of a
classroom event. .

Provided training and monitoring to case management staff to ensure
they were in compliance with the requirements of the program.
Worked as a family case mgr. / social worker for the families of the
head start program. Completed assessments on the families and
developed plans to help assist them the best way possible.
Completed continuous follow-up with the families and tracked their
progress towards goals put in place. Also provided referrals to several
resources for the families such as our on call psychologist which was
available for free with the program, job openings in the area,
information on getting enrolled into college and improving parenting
skills. Provided counseling and vocational guidance with the families
as needed. As a part of my job duties, managed records, assured
health records were up to date and the enclosed information
remained in compliance with the head start standards. Home visits
were also completed during the year to check in on parents and
discuss new information if needed with parents.

substance abuse, and issues with parenting. Sent referrals as
needed to resources in the area to help families get assistance.
Another one of my job functions included helping stabilize the living
arrangements, promote reunification or prevent the utilization of outof-home placement. Provided therapeutic interventions in the home
to support and guide consumers and their families in all areas of
functional domains. Followed up with families to track their progress
and see if plans needed to be modified. Assisted team members
including therapist in continuing quality improvement program for
ongoing systematic evaluation of clients treatment plan. As a
requirement by the company, entered documentation/ notes in
company online database system.

mental disorders with reaching their goals, improving their behavior,
and teaching them new skills to be able to function in everyday life.
Made weekly home visits and developed activities to work on with
clients to focus in on any problem area they may have such as
behavioral issues. Recorded notes and document data into the
computer and update records monthly. Constant follow-up and
monitoring was done on each client until they were able to be exited
from the program.
