40-70 MILE NON-RESIDENT MEMBERSHIP APPLICATION

I wish to make application for membership in the Idabel Country Club. If I am approved for membership, I will abide by
the By-Laws of the Club. 40-70 MILE NON-RESIDENT MEMBERSHIP - Annual Dues $759.90 includes sales tax

Article II Sec. 15 of the IDABEL COUNTRY CLUB BY-LAWS states as follows: Forty to Seventy (40-70) mile non-
resident members shall reside more than forty (40) miles and less than seventy (70) miles from Idabel Country Club for at
least nine (9) months of each year and shall be at least eighteen (18) years of age. (Amended 4/06)

Article II Sec. 6 of the IDABEL COUNTRY CLUB BY-LAWS states as follows: Non-resident and provisional members
shall be entitled to limited privileges of the Club, which is defined as all privileges of the Club except such member shall
not have the right to vote in election nor have any voice in the control of management of the Club.

Article III Sec. 2 of the IDABEL COUNTRY CLUB BY-LAWS states as follows: Forty to Seventy (40-70) mile dues
must be paid in full at the time of application with one exception, if two forms of guaranteed payment are given in the
form of bank draft information and/or credit card. First payment of half to be due on application (only if draft information
and/or credit card information are attached) and the other half to be drafted or charged to credit card on the first day of the
following month, otherwise the full amount is due with the application. For renewing forty to seventy (40-70) members
the drafts will be made on January 1st and April 1st. (Amended 4/06)

A check for at least half the amount of the fee must accompany this application. The Board of Directors will meet to
approve or disapprove this application. If disapproved, all fees will be refunded. If approved, the other half will be drafted
or charged to your credit card on the first day of the following month. Membership runs from January 1 to December 31.
