Media Use and Privacy and Content Advisory:

Students in all theatre classes at CMMS will often view relevant and age-appropriate media from
various film, television, and internet sources. Video recordings of in-class performances may be used
in evaluating student work. Photos of student performances may be posted on the CMMS and/or
class website and Facebook page. If students or parents do not want photos posted, they may
indicate below.

I give the students choice in a lot of what they do, however, I encourage and only allow G and PG
rated performances in my class. Because of the amount of theatre performances and writings, it is
not possible for me to screen everything your student may read. When choosing a monologue and
scene your student may come across language or things they or you as parents consider to be
inappropriate. If they would still like to perform the scene or monologue, they may cut or change
the words to be more appropriate. I will never make the students perform something they are not
comfortable with. Please talk to me if the student, you or any member of the group is having issues.

Contract:
-I give permission for my student to watch G and PG-rated movies or clips _____Yes _____No
-I give permission for photos or videos of my students performances to be posted on the CMMS
and/or class website and Facebook page. _____Yes _____No
-I am willing to arrange for appropriate Performance/Audition Clothing ____Yes ______No
