A G R E E M E N T

I, ________________________________, hereby voluntary agree to undergo Industry Internship Program


which is an academic requirement in the Bachelor of Science in
_____________________________________ degree of Pamantasan ng Lungsod ng Maynila (PLM) under
the following terms and condition:

a. That I shall undergo the said program this Summer, School Year 2015-2016 from APRIL 11, 2016
until the 240 hours is accomplished;

b. That I shall complete the required Two-Hundred Forty (240) Hours before the start of the next
School term with the schedule 8:00AM to 5:00pm MONDAY to FRIDAY and 9:00AM to
3:00PM SATURDAY from APRIL 11, 2016 to MAY 31, 2016;

c. That hours of service shall be credited only upon execution of this agreement and receipt by the
College of a Company Acceptance Letter;

d. That I shall not put the name of PLM in bad light for any misconduct;

e. That PLM may discontinue my participation in the program on reasonable grounds upon notice;

f. That I renounce and waive my claim against PLM for any injury that I may sustain or loss that I
may suffer, personal or pecuniary in the performance of my duties and functions while under
the program; and

g. That I shall submit all requirements within one (1) week after my last day with the company.
