AFFIDAVIT OF LOSS

I, MARIA FRANCES ANTONIETTE T. PEREZ, of legal age,


Filipino, single, and a resident of Blk 36 Lot 32 Joshua Street, Emily
Homes, Cabantian, Davao City, under oath states that:

1. I am a licensed pharmacist, with Registration No. 0074089,


having passed the Pharmacy Licensure Exam last March 2017;

2. Sometime on September 4, 2017 while I was preparing two


(2) valid identification cards for the purpose of obtaining an NBI clearance,
I found out that my PRC ID was missing from my wallet;

3. I tried to locate said identification card among my personal


things but, to my dismay, I was still not able to find it;

4. Despite diligent efforts exerted to locate or retrieve it,


however, all were in vain;

5. That I have given up in looking for my PRC ID, hence, for all
legal intents and purposes, it is now deemed lost and beyond recovery;

6. That I am executing this affidavit to attest to the truth of the


foregoing statements. I am executing this Affidavit of Loss to support my
application in getting a new PRC ID at PRC Davao, as well as for all lawful
intents and purposes this may serve.

IN WITNESS WHEREOF, I have hereunto set my hand this 5th day of


September 2017 at Davao City, Philippines.
