Guidelines Manual: Title: Employee For Regularization GU-ADM-01
1. On the fifth month of probationary period or before the expiration of six (6) months
period of probation, or as the case maybe. The immediate superior of the newly
hired employee shall conduct an evaluation of performance for such period using
the Performance Appraisal form.
2. The standards of evaluation shall be made known to him on the very first day he
reports to work. If he passed the evaluation, he will be recommended for
permanency or regularization and if not, he should be informed a month prior to the
end of his contract.
3. Performance Appraisal form shall be submitted to Head office for HR/Admin group
concurrence, upon confirmation of permanent status, the respective HR/Admin
group shall prepare Personnel Action Notice (PAN) together with the Regular
Employment contract signed by the Department/Branch Manager and all
concerned officers to be submitted to Head office for final approval of the General
Manager.
