A. CHECK THE BASE OF THE STRUTURE OF SCAFFOLD YES NO

1 Is ground level and firm or properly compacted or proper supports have been placed under the
structure?
2 Are base plates adjusted to fit in scaffolding?

3 Are plywood and/or steel box located underneath the base plate and in good conditions?

4 Is the base away from excavation, drain covers, manholes, etc..?

B. CHECK THE STRUCTURE


5 Is horizontal tubular brace fitted every 2 stories of scaffolding?

6 Is X tubular brace fixed every 4 stories?

7 Are adequate bracing provided?

8 Are all the scaffold, board, tubular and staircase in good conditions, no bends or cuts?

9 Is scaffolding system properly anchored to a building or proper supports as required?

C. CHECK THE WORKING PLATFORM


10 Does working platform have three boards and adequate steel kickboards, no gaps between the
boards?
11 Does working platform have strong enough to support the load?

12 Are boards and steel kickboards in good conditions and secured?

13 Are top and mid rails provided above the platform and secured?

D. CHECK THE ACCESS


14 Are the existing access ways (staircases, walkway) left clear?

15 Is access to building properly provided with 3 boards, top and mid raids and secured?

16 Is mid rails provided for each staircase and secured?

17 Is a rest platform properly provided by every story of scaffolding?

E. CHECK THE NETTING


18 Is vertical safety net in place and in good condition, higher than working floor by at least two
stories and secured?
19 Is horizontal safety net in place and in good condition? Is this system strong enough to support
the load and secured?
20 Is perimeter debris net in place is good condition? Is this system strong enough to support the
load and secured?
