RCP Lab Analysis Qaqc Certification Form

This document is a certification form for laboratories analyzing samples using Connecticut Department of Energy and Environmental Protection (CTDEP) Reasonable Confidence Protocols (RCP) methods. The form requires the laboratory to confirm that all specified quality assurance and quality control procedures were followed for each analytical method used. This includes meeting method requirements for preservation, holding times, performance criteria, reporting limits, and analyte lists. The laboratory must also confirm that samples were received in good condition and at the appropriate temperature. Any exceptions must be explained in an attached narrative. The authorized laboratory representative must then sign the form to certify the accuracy and completeness of the reported information.
