AFFIDAVIT OF DISCREPANCY
That I, MARIA LOURDES MERCEDES E. CANLAS (formerly
MARIA LOURDES MERCEDES C. ESCALONA), of legal age, married,
Filipino citizen, and with residence and with postal address at Barangay
Caramutan, La Paz, Tarlac, Philippines, do hereby depose and say:

1. That I am married to Fidel M. Canlas Jr., a copy of our Marriage


Contract is hereto attached as ANNEX “A”;

2. That in our Certificate of Marriage, my first name was inaccurately


entered as ‘‘MA. LOURDES MERCEDES” instead of ‘‘MARIA
LOURDES MERCEDES” which is my true and correct first name as
reflected in my Certificate of Live Birth, a copy of which is hereto
attached as ANNEX “B” ;

3. That the discrepancy between my first name as inaccurately entered


in our Certificate of Marriage and my first name as correctly entered
in my Certificate of Live Birth was due to the unnecessary
abbreviation thereof from “MARIA” to “MA.”;

4. That I am executing this affidavit to attest to the truth the foregoing


declarations and more specifically to show that my correct first name
is ‘‘MARIA LOURDES MERCEDES” and not ‘‘MA. LOURDES
MERCEDES”.

IN WITNESS WHEREOF, I have hereunto set my hand this
