AFFIDAVIT OF LOSS

I, ISAGANI M. DIONELA, of legal age, Filipino, married,


with residential address at #2 Virgo Street, Carmel 5
Subdivision, Tandang Sora, Quezon City, subscribing under
oath, hereby depose and state:

1. That I am the holder of a valid Integrated Bar of the


Philippines (IBP) identification card issued by the Integrated
Bar of the Philippines;

2. That last February 15, 2018, I placed the aforesaid IBP


identification card inside my bag while I am at the Quezon City
Hall of Justice;

3. That when I looked for the IBP identification card


inside my bag, I could no longer locate it;

4. That my said IBP identification card could not be


found notwithstanding diligent efforts to locate the same and
is now considered lost for all legal intents and purposes;

5. That I certify this oath to report the loss of my IBP


identification card issued by the Integrated Bar of the
Philippines to avoid the said missing IBP identification card
from being used for illegal purposes by other persons;

6. That I execute this affidavit to attest to the truth of all


the foregoing facts and for securing a new IBP identification
card.
