Each letter you send should be sent via Certified Mail and Return receipt. Fill out the green Card.
Double click on the Certified Mail above and it will allow you to replace the 1234 5678 etc. number
with the one on your green receipt form. Before you print this document make sure you delete this
tutorial as well as the title of the document. All yellow highlighted areas should be replaced with your
pertinent info. At the end of each document you will find a CC to The Federal Trade Commission and
the State Attorney General. The FTC info can remain the same, however you will need to replace the
Attorney General information with that of your own State Attorney General. You can find it by
googling ie: Oregon Attorney General for example.
Once you have personalized the letter, Unhighlight the yellow areas of the document and delete the
tutorial and title of the document. Areas you will need to personalize with your information will be
highlighted in Yellow. Although there is a CC to the FTC and Attorney General you do not need to
send copies to them in my opinion. Just having a CC is good enough to make the Bureaus or
Collection agencies believe that you are sending copies to those organizations. You need to think like
a collection agency or creditor.. they work on 90% intimidation and 10% rule of Law.
Delete this heading and replace highlighted portions with your information
before printing, and eliminate yellow highlight.

I have recently obtained a credit report from (credit bureau). It shows the above account with your
company was (days late, charged-off, other problems, etc.) To the best of my knowledge, I have
always paid this account promptly and satisfactorily. This incorrect information is seriously hurting
my credit rating, and I would appreciate it if you would verify this information and correct it with all
three credit bureaus. If the information cannot be verified, please delete the account from my credit
report.
Please inform me with the results of your verification and please provide me with proof of that
verification along with a signed sworn statement that this account was in fact mine.
