Brgy. xx, Tacloban City do hereby NAME, APPOINT AND CONSTITUTE, PATERNO
A. CANDELA, of legal age, Filipino, married and a resident of Gumamela St., Brgy. xx,
Bañesville, Tacloban City to be my true and lawful attorney-in-fact, for my name, place
and stead, to do and perform all or any of the following acts, namely:

1. To transact, apply, follow-up, and pay necessary fees for the renewal of
franchise of my motor vehicle bearing Plate No. SSSxxx from the Land
Transportation Franchising Regulatory Board (LTFRB) and Land
Transportation Office (LTO);

2. To obtain a confirmation of franchise for said motor vehicle;

3. To sign, execute and deliver any document which may be required in relation
to the purposes hereof.

4. To do such other acts and things which may be required or necessary in the
accomplishments of the aforesaid powers.

HEREOF GIVING AND GRANTING unto our said Attorney in fact full powers
and authority whatsoever requisite or necessary or proper to be done in about the
premises as fully to all intents and purposes as I might or could lawfully do if personally
present and hereby ratifying and confirming all that our said Attorney in fact shall
lawfully do or cause to be done under and by virtue of these presents.
