I acknowledge receipt of my 10-digit Personal Banking number (PBN) ____________________ and 6-digit Personal Identification
Number (PIN) for PhoneBanking.

Third Party Payment Facility

I hereby authorise you to activate the third party payment facility from my accounts linked currently, and those that are linked in future, to
my PBN or my Internet Banking ID with options marked below:

I agree that I am entitled to instruct the bank to transfer funds over PhoneBanking/Internet Banking, as specified, from my accounts
linked to the above PBN to the aforesaid beneficiary(s) only up to a total of Rs 250,000 per day (from 00:01 hours to 24:00 hours Indian
Standard Time).

Note: For Internet Banking, kindly note that pre designated payments may only be made to beneficiaries with accounts with HSBC in
India for Resident accounts and additionally by way of Overseas Transfers for customers debiting their NRE accounts.

Option II General Third Party Payment on PhoneBanking Yes / No

I agree that I will be entitled to instruct the bank to transfer a total amount not exceeding a total of Rs 50,000 on any day (from
00:01 hours to 24:00 hours Indian Standard Time) from my accounts linked to the above mentioned PBN to any beneficiary/s.

Option II General Third Party Payment on Internet Banking (online@hsbc ) Yes / No

I agree that I will be entitled to transfer a total amount Rs _______________ (not exceeding Rs.50,000) from my accounts linked to the
above mentioned Internet Banking ID on any day (00:01 hours to 24:00 hours Indian Standard Time) to any third party beneficiary/s
that I specify while using Internet Banking (online@hsbc).

I have read the Internet Banking service (online@hsbc) and PhoneBanking terms and conditions and agree to be bound by them. I
understand that usage of PBN and PIN for accessing the PhoneBanking service will be construed as acceptance of PhoneBanking terms
and conditions and registering for the use of Internet banking service (online@hsbc) will be construed as acceptance of Internet Banking
(online@hsbc) terms and conditions. I further agree that the bank shall be under no duty to verify the identity or authority of the person
giving any instruction or the authenticity of such instruction apart from verifying my PBN / Internet Banking ID and my PhoneBanking
PIN / Internet Banking password, as the case may be.

I also understand that the 3rd party payment facility on Internet Banking can be activated only after I register for the Internet Banking
service at our website. I also understand that if online registration is not completed within a month of sending this form, then this third
party payment activation request for Internet Banking will become void.

I also understand that the 3rd party payment facility on Internet Banking will be automatically cancelled by the Bank in case this facility
remains unused for 12 months.
