Self- Rating Questionnaire


Instructions:
This questionnaire consist of 55 brief statements. Read each statement very carefully and decide
how much each statement very carefully and decide how much each statement describes you by using
the rating given below. Your honesty will greatly affect your overall ratings, so just rate each statement
as truthfully as you can. Remember that nobody ever does everything very well. This exercise is
designed to help you identify the areas where you can develop.

Use the assigned numbers below to indicate how much each statements describes you:
