PETITION
The undersigned petitioner, (your name), unto this Honorable Court
respectfully states that:
1. She is a Filipino citizen, single, over twenty one (21) years old,
and a resident of Tagbilaran City, copies of the picture of
petitioner with signature at the bottom part are hereto attached and
marked as Annex “A”, “A-1” and “A-2”;
2. She has completed her law studies at (your school);
3. She has been admitted to the practice of law in the Philippines as
shown by the Certificate of Membership in the Philippine Bar,
which contains the Petitioner’s Roll of Attorney No. (your roll #).
A copy of the Certificate of Admission to the Philippine Bar is
attached and marked as Annex “B” hereof;
4. She is a bona fide member in good standing of the Integrated Bar
of the Philippines, Bohol Chapter, attached is a copy of her
Certificate of Good Standing as Annex “C”, and has paid her
membership due as per IBP Official Receipt No. 1042200 dated
(date), a photocopy of which is attached as Annex “D”;

5. She has also paid the Professional Tax Receipt due her as
evidenced by PTR No. 0332908 dated (date), a photocopy of
which is also attached herewith as Annex “E”;
6. She has not been accused of any crime involving moral turpitude
before any court or tribunal in the Philippines;
7. She possesses all the qualification and none of the
disqualifications of a Notary Public;
8. She has not applied for commission before any other jurisdiction;
9. She undertakes to inform the Executive Judge within ten (10) days
in case of any change in the given address and to submit her
monthly report religiously;
10. She has attached herewith proof of payment for the filing of this
petition as required by the Rules marked as Annex “F”;
11. She now desires to be granted Notarial Commission as Notary
Public for and in the City of Tagbilaran.
PRAYER
WHEREFORE, in the view of the foregoing, it is respectfully
prayed of this Honorable Court, after due notice, publication and hearing to
grant this Petition for Appointment of Notarial Commission of the herein
Petitioner for and in the City of Tagbilaran.
Other just and equitable reliefs are likewise prayed for.

action before the Supreme Court, Court of Appeals, or before any court,
tribunal or agency concerned; and should I learn of any similar cause of action
filed before any of the aforesaid bodies, I undertake to notify this Honorable
Court within five (5) days from notice.
