The learner's needs, progress and achievement card allows recording of details of concerns reported, agreed resolutions, and signatures. The parents/guardians communication card similarly tracks details of encounters, concerns, resolutions, and signatures. Both forms are meant to document engagement and issues over multiple quarters and school years.
