Self Final — Chase Cochran Compared to my mid term I feel like I have improved slightly in this category. I felt like I never really
ever put a patient, or others, in a dangerous situation but I can recall multiple times thinking about certain
situations that required balance to ensure that the patient felt safe and that I felt the patient was safe. For
example, having patients hold onto the treadmill (for its height) or the counter to allow for them to not
have to challenge their balance as much and make and exercise such as the SL heel raise more safe. I feel
like I can consistently perform this on my own.

Self Final — Chase Cochran I feel that I do a very good job of remaining professional during all times such as dressing appropriately,
wearing my name tag, maintaining professional relationship with my coworkers, and even when a patient
says something that is not appropriate given the setting and situation. For example, a patient said
something inappropriate and I did not know how to respond exactly so I just ignored the comment. They
said something about their sex life as I was discussing their diagnosis with them. It did not pertain to PT
as it was meant to be a joke. I thought the best thing to do was to ignore this then if it became a
reoccurring issue, I would address it. It has not since that situation has occurred so I am hoping they
either caught the hint or they realized what they did.

Self Final — Chase Cochran I feel that I do a good job of holding myself accountable for my actions and mistakes. I understand that
when I make a mistake I need to reflect back upon it and take accountability for that mistake. I may need
guidance from my CI in terms of helping me realize what my mistake was but I have no issue what so
ever to admit when I made a mistake and afterwards I out forth my best ability to not make the same
mistake again. Because I understand what my level of education is right now and know there is no way I
am going to be perfect, I will make many many mistakes for forever, so holding myself accountable is a
thing I do well.

Self Final — Chase Cochran My communication both verbally and nonverbally I believe is a strong suit of mine. I feel like I do a
good job of explaining things to patients in a simple way, communicating with the technician to help me
with my patients (this I have gotten better at), and communicating with my CI whenever I need help or
even when I think something should be changed. I also listen very actively and maintain engagement in
all interactions, with direct eye contact when appropriate and doing my best to hold the eye contact as
much as I can while documenting.

Self Final — Chase Cochran I still have not been put in any situation where I have had to deal with any patient with cultural
differences, values, preferences, and needs. I still feel I would do very well in the category if I were in
this situation. We see a lot of different types of patients at Concentra, but no one really has spoken to me
about specific cultural and religious differences or specifics they would like me to cater to.

about modesty (closes curtain when taking off/on electrodes for females on low back etc)

Self Final — Chase Cochran I still feel I do this often. I feel like I know what I am good at and what I need to work on exactly due to
my self reflection that I do. As much as I try to live my life outside of the clinic I often get anxiety outed
elf the clinic and think about the things I did the day of or the week of. I think about what I could've done
differently, if I missed anything, and so on. I feel like I create self short and long term goals and I do my
best to achieve these. I also know when to seek guidance to address my weaknesses as I may not see all
the weaknesses I have or may need a helping hand to address them.

of struggle and asks for additional practice and asks for feedback for growth.

Self Final — Chase Cochran I feel like compared to my midterm I am doing a better job at clinical reasoning. Having a reason behind
why I am doing a certain treatment or prescribing a certain excess, or at least obtaining documentable
objective information as to why I am providing a specific intervention. For example, instead of just
giving a patient a hamstring stretch because they have knee pain, but putting them through a 90/90
hamstring MLT and obtaining data that supports why I am providing that treatment, and I recognize how
this will be effective for my prognosis and POC of the patient.
CI Mid-Term — leah stefanou Chase is starting to develop his clinical reasoning skills. He needs more practice when performing initial
evaluations to identify what the root issue is, main issues are and what to start treating first. He does a
great job with progressing patients throughout the course of case, however needs more practice with first
day interventions which will only come with more time/experience with patients.
CI Final — leah stefanou Chase is becoming more proficient in his clinical reasoning abilities. He is very good with more standard
and simple MS diagnosis, and is becoming more proficient with complex diagnosis.

Self Final — Chase Cochran I feel like I do a good job of screening although I would like to still get better at this so I want to improve
in this a lot still. I still need a decent amount of guidance as to what I should screen for if the case is more
complex. I do a good job of reviewing past medical history to help with my creation of my POC or of my
evaluation as well as looking at other sources that may help with this. This is very accessible through the
notes that I can view so I do this often, especially if it's a complex patient or a new patient. I still need a
lot of help as to which pathologies and deficits I should choose to screen for though.
CI Mid-Term — leah stefanou Chase is doing a great job implementing his test/measures he has learned in school to actual patients in
the clinic. He researches special testing/goniometry measures prior to evals to ensure accuracy.
CI Final — leah stefanou Chase does a good preliminary job screening patients, however needs to continue to think about whole
health of the patient in terms of healing and how that effects prognosis.

Self Final — Chase Cochran I can see great growth in my examination confidence and effectiveness. I feel like I can perform an eval
under no supervision on many cases unless they are very complex, then I have a tough time sometimes as
to what to screen for and what to rule out. I would require a decent amount of guidance in these
scenarios. I create my own outlines given the case and make sure I address everything I should need to in
term of tests and measures but I still required some guidance if I get lost.
CI Mid-Term — leah stefanou Does a great job of collecting basic information including subjective/objective information during
evaluation, more practice needed to improve only comes after repetition with experience.
CI Final — leah stefanou Chase is becoming more fluid with implementing his evaluations. He is leaning how to better perform
complex evaluations in a structured way.

10. Patient Management – Evaluation
Self Mid-Term — Chase Cochran I always priced my initial evaluation with evaluation of the patients notes from other health care
professionals, and they diagnostic imaging to ensure myself as having the most information possible
before going into the evaluation. Once I am in the emulation I obtain the data I believe is the most
important and then use this to make my clinical judgements. If I obtain all the information and then go to
make my clinical judge meant and feel lost then I consult with my CI to ensure I am doing a good job or
if I am not on the correct track.
Self Final — Chase Cochran I use correct clinical judgements based off of examination history as I listed above, such having a reason
behind why I am doing a certain treatment or prescribing a certain exercise, or obtaining documentable
objective information as to why I am providing a specific intervention. I use this information to then
create my plan or interventions and exercises the prescribe to the patient. This still falls in the lines of me
reburies some guidance and supervision to do so correctly sometimes, if its a complex case, but overall I
feel I am at the intermediate stage for this.
CI Mid-Term — leah stefanou Chase does a good job of basic evaluation skills. Needs more analysis of whole body health/age/gender
to more specifically predict prognosis etc.

Self Final — Chase Cochran I see very few super complex cases so I feel in terms of determining a diagnosis, I do a good job. I have
seen a lot go growth in this as I try not to get tunnel vision when reading the patient history and MOI
from the doctors note but I keep an open mind during my evaluation to prove an adequate diagnosis. I
still require a decent amount of assistance during this, but have gotten a lot better, especially at more
simple cases, just understanding that a joint can be irritated, or it can be a simple muscle strain because I
see 90% acute cases.
CI Mid-Term — leah stefanou see above- good basic development of diagnosis and prognosis however needs to additionally think about
job demands, degenerative changes, co-morbidities etc for accurate prediction of rehab
CI Final — leah stefanou Chase is improving with consideration of co-morbidity and current health status and how that will effect
a patients prognosis in addition to thinking about job title/responsibilities.

Self Final — Chase Cochran Compared to the midterm I feel like there wasn't a lot of time for growth in this category given that I
never have prescribed a plan of care that would even be close of dangerous, or not effective for the
patient. I have tried my best to create the most effective POC that I can in my mind and then seeking out
help or getting confirmation that my POC is sufficient for the patient. My CI does a great job at assisting
me with this but also challenging me to try my best to do the on my own. I do a good job of prescribing
specific impairment and functional goals (mock definitely over prepared me for this), but I feel like my
POC is generally a strong suit of mine if the case is not too complex.

Self Final — Chase Cochran This is a huge growth from day one for me. Mainly just confidence wise but also actually feeling and
assessing joint play correctly, tissue texture and pliability correctly, and so on. This has come with just
practice, practice, and more practice. I communicated with my CI that I would like to feel as many
shoulders, low backs, knees, etc as possible just to know what is and what is not hypo mobile. I feel like I
am getting a lot better at administering both STM and joint mobs effectively and always obtaining
feedback when possible. I still need to completely understand when its appropriate to apply a grade 3
joint mob vs. when should I not fr example but other than that I have seen huge growth in this, especially
from day one but even from midterm.
CI Mid-Term — leah stefanou All procedures selected are appropriate. Needs more experience in general to gain experience.
CI Final — leah stefanou Chase is very good at demonstrating appropriate exercises/interventions and is improving in his more
complex manual therapy skills. He is working on becoming more sensitive and learning how to identify
what he is feeling though that sensitivity.

Self Final — Chase Cochran As compared to the midterm I feel like I have gotten better at explaining what needs to be explained to
patients. In terms of their diagnosis, and why they are feeling the pain they are feeling. I would like to get
better at describing pain science to patients, and what exactly is pain and how the brain influences pain,
or at least get better at learning the correct way to do so. I feel like I do a good job of modifying and
determining what type of style best fits the patient as well as using 5th grade language to ensure that they
undertsdn it but then educating them beyond 5th grade level afterwards if I feel it is appropriate.

Self Final — Chase Cochran Documentation is once again still a strong suit. I would like to get better and see more examples of how
other therapists explain and document certain things such as tissue pliability or tension just to get another
point of view of this. But I feel like I do a very good job of documenting appropriately and thoroughly
keeping in mind that I will be leaving soon so making sure my notes are 100% repeatable for my CI to
perform if she is confuse don previous treatments. I have not had a note late and have stayed late a
couple times if I am required to do so, so I can get my notes done.
CI Mid-Term — leah stefanou Excellent documentation skills, specifically with assessment/follow up plans at next visit. Objective
documentation is very good, can be more creative in terms of muscle/joint play or postural assessment
etc when struggling to come up with a statement/findings.

This determines how much, how far, and how often and the type of technique used for the patients work
related tasks. I then use that information given from the patient on the assessment to create functional

when necessary and doing a good job of ensuring that I document this correctly both in the note and in

Self Final — Chase Cochran The only other interaction I have in terms of personnel other than my CI is the technician. I feel that I
give her a good explanation (as well as to my CI) of what I need of them when I need their assistance. I
ensure that it is appropriate, and make sure they have no questions before I leave them to go to perform
whatever task I must.
CI Mid-Term — leah stefanou Good time management with patients and can start to handle more then 1 patient at a time. Needs to work
more closely with PT tech to delegate roles to transition from being a PT tech to the PT. Continue to
work on time management of patient within the appointment time and scheduling.

managing complex evals (thinking about what to do on day 1 and progression from there and also
consider making a self template for complex evals mostly spine so you dont have to come up with 1 just
prior to eval, might improve organization/fluidity of process).
Caseload/Other Comments
1) What is the full caseload at your clinical site for a new graduate?
2) Considering the rating anchors, what percent of a new graduate caseload is the student capable of managing at this
time?
Self Mid-Term — Chase Cochran My CI sees around 16-21 patients a day. Currently I am at around 5-8 a day. So around 30-40%
Self Final — Chase Cochran 1) My CI sees around 16-21 patients a day. I have been seeing no less than 5 currently and at a max of
around 9. This will increase next week (final week) due to my CI being gone and me taking on a
somewhat larger role. 2) I am around 40-50% depending on the day.
CI Mid-Term — leah stefanou Caseload is appropriate for right now, (ranges 4-6 per day) with help from PT. I think at max he should
be seeing no more then 8-10 per day with help from PT by the end of this clinical.
CI Final — leah stefanou Caseload still appropriate, however become more comfortable with it as patients increase to 8-10 per day.
