INCIDENT INVESTIGATION REPORT

Date of Incident: 11/06/2019

Time: 6.15 P.M

Shift: GENERAL

Location: STACKER RECLAIMER AREA

Department: RMHS

Brief Description:
Mr Bikash Ch. Sahu having Gate pass No: V170063, was working at Reclaimer- 4
for maintenance job of Air conditioner. After completed his job, he was
descending from Reclaimer. At that time his right leg foot (angle)got injured by
trapped with movable handrail part of that stacker.
Immediate Action
He was taken to First-aid center, Tata steel, BSL by the Supervisor ;As
advised by the doctor, he was referred to Samal care Aswini emergency
centre (time- 8.45P.M), where his foot was X-rayed. His left foot
sustained hairline crack.
Preliminary Investigation findings.
1. He along with helper descending from stacker platform. He got
injured while that stacker was suddenly moved.
2. He was wearing safety shoes.
3. Work permit and TBT was existing.
4. SOP was existing.
Root Cause.
1. Stacker was in running condition.
2. The workmen was unknown about the hazards involve in that place .
4. Carelessness and absent minded behavior of employee.

Area of incident

Corrective Action
1. The area was investigated and workmen were counseled.

2. Advised to learn work place hazard before work in that place.

3. Regarding the incident on the spot supervisor was issued a warning letter for
his carelessness.

Preventive Action
1. Awareness regarding six directional hazards wherever necessary shall be done.

2. Unknown or unauthorized person are strictly avoided in that place.

3. Job should carried out after well knowing about that area hazards.

4. Educating the workforce in tools box talk regarding the management of such
jobs to avoid accidents and the details of the work shall be explained and discussed
in detail so as to avoid recurrence.

5. Six directional hazards shall be identified fully and corrective action shall be
taken.

6. The incident was shared amongst supervisors and workers and strictly advised
to report any such incidents, accidents or near miss immediately.

7. Job cycle check wherever such critical jobs have been carried out shall be
undertaken by senior management to assess the effectiveness of SOP and prevent
recurrence.

9. Educating the worker the need for self safety and the process of such activities
to avoid any personnel injury.

We fully accept that it was an unfortunate incident and it is a failure on our


employees part in execution of job.
