We would like to humbly request your Honorable Office for an EXEMPTION FROM
THE NUMBER CODING SCHEME.
On June 2018 last year, I was diagnosed with Ascending Aortic Aneurysm, a medical
condition where there is an abnormal bulging and weakening of aorta at the point before the
curve. If an aortic aneurysm ruptures, it can cause life-threatening bleeding.
Just this year, after medical examinations, it was discovered that my aortic aneurysm
grew to the size of a tennis ball which could rupture anytime and could cause my death.
Thus, with the help of beloved relatives and friends, I underwent heart surgery called
“Modified Bentall Procedure” last 01 July 2019.
As of today, I was already discharged from the hospital and en route to recovery. I
am the breadwinner of my family—a husband to my wife and a father to two beautiful
children, thus, I am striving to get back to work and lead a normal life. However,
considering my perpetual condition and the fact I am still recovering from my heart surgery,
it would be risky for me to take the commute to and from the hospital for my check-ups and
to and from my workplace.
As I only have one (1) car, we therefore humbly and most respectfully request your
kind office for an exemption from the number coding scheme. The details of my car are
provided in the OR and CR attached to this request.
We hope this fulfills your requirement for the GRANT of EXEMPTION FROM THE
NUMBER CODING SCHEME. I am hoping for your prompt and preferential response to
my request.
