The child who has a ained majority is required to sign the request form and get his signature a ested by either :
1. The Donor who has invested in the scheme in the name of the child supported with a photo ID proof of the child OR
2. The bank which is / is to be registered in the folio supported with a photo ID proof and any of the documents
men oned in form sec on F-2
The documents to be submi ed along with the Form towards a esta on are:
a) Signature a ested by Donor
i) Photo ID proof of the child
b) Signature a ested by the Bank
i) Photo ID proof of the child
ii) Any one of the suppor ng documents as men oned on the reverse

The form is complete in all respects.
The form is signed by the unit holder i.e. minor who has a ained majority & has been a ested by the Donor or
Bank as opted by the investor
Copy of proofs as men oned in the form are a ached
