I, DENNIS W. DULNUAN, of legal age, single/married, Filipino


Citizen, with residence and postal address at Pico, La Trinidad, Benguet,
Philippines, under oath declare and state that:

1. I am the registered owner of that motor vehicle particularly


described as follows: Make/Series: Mitsubishi/Fuso; Year Model:
1992; Plate No.: DKL859; Body Type: Jitney; Engine No.: 4D33-
908521; MV File No.: 0472-00000059345;

2. The color of the said motor vehicle was not indicated in the Official
Receipt (OR) issued by the Land Transportation Office (LTO);

3. The actual color of the said motor vehicle is MULTI COLOR;

4. I am executing this Affidavit in support of my request for the


indication of the said color (MULTI COLOR) in the Certificate of
Registration and Official Receipt pertaining to the above-
described motor vehicle;

5. Finally, this Affidavit is being executed to attest to the truth of the


foregoing statements and for all legal intents and purposes it may
well serve.

IN WITNESS WHEREOF, I have hereunto affixed my signature this
