Dear SDS Mascareñas:

Greetings from Teacher Education Council!

We are currently conducting a national survey which shall serve as a basis for the
development and implementation of a training program in democratic citizenship. We
shall be gathering data from in-service teachers of selected public schools across all grade
levels and regions in the country. We are requesting for your support and assistance in
mobilizing the public school teachers under your supervision in accomplishing the
instrument through Google form. The survey revolves around your democratic ideals,
values and principles as well as your civic engagement. This will take approximately 10-
15 minutes to accomplish. It can be accessed through the link:
http://deped.in/bayanihan. The survey includes, among others, a set of items that are
assessed with a rating scale.

In completing the survey, please be guided by the following:


(1) Have your electronic mail account and password ready. Type the link in address
bar of your browser and log in using your e-mail account.
(2) Accomplish the online survey within the period of November 04 – December 06,
2019.
(3) Answer the survey honestly. Do not leave any item unanswered.
(4) Multiple submissions are not allowed.
(5) When you’re done, do not forget to click the SUBMIT button. An acknowledgment
message shall be sent to your e-mail.
(6) For technical concerns, kindly inform us immediately through e-mail
(Vdemocracy2019@gmail.com).
