I, CARLO MATA, Filipino, of legal age, with office address at 7th Floor,
Tower II, Enterprise Center, 6766 Ayala Avenue, Brgy. San Antonio Makati City,
after having being duly sworn in accordance with law do hereby depose and say
that –

1. I am the duly elected and incumbent Corporate Secretary of DSM


Damayan Sa Manila Foundation, Inc. (“DSM”), a corporation organized and
existing under the laws of the Republic of the Philippines, and with principal office
7th Floor, Tower II, Enterprise Center, 6766 Ayala Avenue, Brgy. San Antonio
Makati City (the “Foundation”).

2. I certify that the Secretary’s Certificate consularized 25 July 2015 (a


copy of which is hereto attached and made integral part hereof as Annex “A”) is
valid, subsisting and has not been revoked to date.

3. I am executing this affidavit to attest to the veracity of the foregoing


facts and for all intents and for whatever legal purpose it may serve.
