Dear dr Cassimatis,
I am writing this letter to refer Mrs Alison a 28 year old teacher by profession with a probable
diagnosis of schizophrenia and its associated disorders for further treatment and assessment.
Mrs Alison has been my patient since 10 years and is a married women with 2 children who lives
with her husband at his parent’s place. She is a non smoker and does not consume alcohol. She also
has never used any mood enhancing drugs for amusement. However, there is a positive family
history associated with schizophrenia that gets controlled by Risperidone.
On her first visit, she complained that she is not enjoying her health in good form with the symptoms
of low grade fever,loss of appetite and weight. She further added that there is a lack of motivation
and interest in her work. However, she appeared almost normal with not a depressive mood having
stable vitals and negative examination findings. Relaxing therapy, temperature charting and regular
exercise were advised at that time.
On next visit, she came up with a mildly depressed mood with no change in the previous symptoms.
She added that it is difficult to concentrate in work and not comfortable living with her husband’s
parents. She further narrated that her mother in law blames her of laziness and is responsible to
turn her husband against her.The provisional diagnosis of depression and schizophrenia was
considered for which she was prescribed Diazepam 10mg at night and paracetamol for her
headaches. Moreover, she was advised to bring her husband on next visit.
On todays’ visit, her husband explained that she is having poor eye contact with people , disturbed
planning and observing hallucinations since last 5 days. She appeared to be depressed with
disoriented speech And awkward behaviour . Rest of the findings were normal.
It Will be highly appreciated if you look into her condition for further management and assessment.
