ADDENDUM TO AN EXISTING EMPLOYMENT AGREEMENT

THIS ADDENDUM TO EMPLOYMENT AGREEMENT (the “Agreement”) was made by and between ____
(the “Company”), with office address at ____ and ___ (“Employee”), with residence address at ____ on
___ 2020 set forth below.

WHEREAS, the parties wish to modify the terms of the Employment Agreement dated ___ as set forth
herein.

NOW THEREFORE, the parties hereby agree that the following constitutes modifications of the terms and
conditions in the Employment Agreement dated ____.

1. Paragraphs 1.1 and 1.2 of Commencement of Employment are deleted and replaced with the
following:

1.1 Position. The Company agrees to employ Employee to act as a ___.

1.2 Term. Your terms and conditions of employment are described in Annex B and such further
duties and responsibilities as the Company may delegate to you from time to time.

2. The Job Title and Paragraph 1 of Annex A are deleted and replaced with the following:

Job Title: (indicate here new title)

1.1 Base Compensation. For all services rendered by Employee under this Employment
Agreement, the Company agrees to pay Employee ___ per month.

3. The Company agrees to give commission to the Employee ___

4. The Parties reaffirm no other terms or conditions of the above mentioned Employment Agreement
not hereby otherwise modified or amended shall be negated or changed as a result of this here stated
addendum.
