The nursing care plan assessed a patient experiencing social isolation due to feelings of indifference towards others. The nursing goals were for the patient to demonstrate willingness to interact with others after a 2-week learning experience. The nursing interventions included establishing a therapeutic relationship with the patient, encouraging participation in group activities, providing positive reinforcement, and helping the patient identify diversional activities to encourage socialization. The outcome criteria was for the patient to independently interact and participate in activities with others after 2 weeks.
