The client presented with difficulty breathing and the mother reported "My child is having trouble breathing". Upon assessment, the client showed restlessness, irritability, tachycardia, cyanosis, diaphoresis, nasal flaring and tachypnea. The diagnosis was impaired gas exchange related to ventilation perfusion imbalance. The plan was to monitor the client's respiratory status and provide bronchodilator therapy to improve ventilation within 1 hour. After interventions, the goals of improved ventilation were met.
