AFFIDAVIT

I PHOOLCHAND son of JAGANNATH MEENA , presently residing

State Bank of India, Madhada branch Ta. Sihor , Dist. Bhavnagar

(Gujarat) (India) hereby state as follows :

My daughter KIRAN was born on 02-12-1999 at Kishanpura, Tehsil:

Bassi, Dist. Jaipur (Rajasthan). I have no documentary proof in support of

my daughter’s place and date of birth.

Due to oversight and by mistake I have not registered the birth date at

the time of birth before competent authority.

I take oath and solemnly declare/ affirm that the particulars furnished

by me above are correct and that I have not concealed or misrepresented any

facts.

Verified on this 16TH day of July of the year 2014 that the contents of

my above affidavit are true correct to the best of my knowledge and belief and

nothing in material has been concealed there from.
