This is to request medical expenses support for FLORENDO DE CASTRO OLARTE, 53


yrs. old of Dao, Tuy, Batangas. His sister approached us to seek for this endorsement. Mr.
Florendo Olarte has allegedly been suffering from St- Elevation myocardial infarction in
congestive heart failure, urinary tract infection, metabolic acidosis, hyperurecemia and cardiac
arrhythmia. Also, he had recently undergone intubation. Since he is separated to his wife and has
no children, his siblings were the one shouldering the medication. The income of the family is
barely enough to support daily expenses.

Florendo D. Olarte requests for support for his treatment. We hope this humble request
merit your kind consideration.
