This document provides a job safety analysis form for changing gas cylinders. It identifies potential hazards for each step of the task and recommended controls to mitigate the hazards. The three main steps are: 1) Transport the full gas cylinder, 2) Replace the empty cylinder with the full one, and 3) Transport the empty cylinder. Hazards identified include cylinders falling over or off carts during transport, gas leaking from improperly closed or damaged valves/regulators, and cylinders exploding due to flame or spark contact. Recommended controls include securing cylinders to carts, checking valves/regulators/washers, and marking empty cylinders.
