JOINT AFFIDAVIT OF TWO


DISINTERESTED PERSONS

We, MELVIN C. ROSALES and EDILBERTO R. BALDOMERO,


both of legal age, Filipinos and residents of Sta. Ana, Pateros, Metro
Manila, under oath depose and say that –

1. We personally know MARIA KRISTINA Saldivar DABO and MA.


KATHRYNA T. SALDIVAR, both residents of Pateros, Metro
Manila and biological children of the late MARNITA T. SALDIVAR.

2. However, on Fortune Life Insurance of the late Marnita T. Saldivar


under Policy No. F100035 and E-37597, their name appeared
thereat as:

- MA. KRISTINA Saldivar DABO and;


- MA. KATHRYN T. SALDIVAR

instead of their true and correct name appearing on their


Certificate of Live Births and Valid IDs as;

- MARIA KRISTINA Saldivar DABO and;


- MA. KATHRYNA T. SALDIVAR

3. We personally know the name and personal circumstances of the


subject persons because we have been their family’s friends and
acquaintances for several years; hence, we have acquired personal
knowledge of their person, parentage and we hereby confirm that:

MARIA KRISTINA Saldivar DABO and MA. KATHRYNA T.


SALDIVAR as appearing on their Certificates of Live Birth and
Valid IDs and MA. KRISTINA Saldivar DABO and MA. KATHRYN
T. SALDIVAR as appearing on Fortune Life Insurance of the late
Marnita T. Saldivar under Policy No. F100035 and E-37597
refers to one and the same persons being MARIA KRISTINA

Saldivar DABO and MA. KATHRYNA T. SALDIVAR as appearing
on their Certificates of Live Birth and Valid IDs. .

4. We are not related to the subject persons or their family neither we


have been offered or received money or financial gain; hence, we
have no reason to state the foregoing instrument falsely.

5. We are executing this affidavit to attest the truth of the foregoing


for all legal intents and purposes.
