I, ESTANISLAO LUCINA CANAPI, of legal age, Filipino, and a resident of #158


Bonifacio St. Centro, Solana, Cagayan after having been duly sworn in accordance with
law, hereby depose and state:

1. That I have filed a complaint against RCCL CREW MANAGEMENT INC./


GERARDO ANTONIO BORROMEO in the National Labor Relations
Commission for Illegal Dismissal;

2. That I have incurred certain costs and expenses in pursuit of the said complaint
such as attorney’s fees, litigation costs and other personal expenses, with the
following breakdown:
Attorney’s fees: $100 USD/ Php5, 000
Transportation expenses: Php4, 000
Food and Hotel Accommodation: Php5,000
Taxi fee: Php400.00

3. That I am demanding the payment of the aforementioned litigation costs by way


of damages from the respondent for the expenses which I have incurred relative
to the filing of the complaint against the said company;

4. That I am executing this affidavit to declare the truth of the foregoing facts and
for whatever legal purpose it may serve.
