Individual Performance Commitment and Review Form (Ipcrf) For Teachers Elementary Level

The document is an Individual Performance Commitment and Review Form (IPCRF) for a teacher during the first semester. It outlines three key result areas (KRAs) that are measured on a scale of 1 to 5: (1) Teaching and Learning Process, (2) Utilization of Learning Resources, and (3) Implementation of Technical Assistance from Observations. The teacher's performance is assessed based on criteria such as preparation of lesson plans, use of instructional materials, and incorporating feedback from school head observations. The ratings and scores determine if the objectives for each KRA are achieved.

Rater Ratee Approving Authority

INSTRUCTIONS:
1. For teachers who have served at least a year of service in DepEd, they must fill-out the two (2) sets of IPCRF (1 st Semester and 2nd Semester). To
come-up with the Overall Rating for SY 2017-2018, they must add-up the initial rating for the two (2) rating periods and divide it by 2, for
example: 4.3 (initial rating for the 1st Sem.); 4.6 (initial rating for the 2nd Sem.):
4.3 + 4.5 = 8.8
Page 17 of 18
8.8/2 = 4.4
4.4 = Final IPCRF rating for SY 2017-2018

2. For newly hired teachers, they have to fill-in the semesters applicable to their ACTUAL date of service. For example:
a. Teacher A, hired August 6, 2017, IPCRF to be filled-in should be both 1 st Semester and 2nd Semester;
b. Teacher B, hired November 8, 2017, IPCRF to be filled-in should be for the 2 nd Semester only, therefore, the initial rating for
the 2nd Semester shall become the FINAL rating.

3. This IPCRF ready-to-print and is consisting of 13 pages. YOU ARE NOT ALLOWED TO MAKE ANY CORRECTIONS/ALTERATIONS/ADDITIONS to the
template. Any comments/suggestions will be treated separately.
