AFFIDAVIT OF CONSENT FOR TRAVEL OF A MINOR ABROAD

We spouses, HARRY J. POTTER and GINEVRA W. POTTER, both of


legal ages, married, Filipinos, presently residing at No. 12, Grimmauld Place,
Baguio City, after having duly been sworn to in accordance with law,
depose and state:

1. That we are the biological and legitimate parents of ALBUS SEVERUS


W. POTTER, who was born on the 31st of August 2008, at Baguio City
and that he is a minor passport applicant;

2. That our child will be travelling to the United Kingdom to pursue his
studies at Hogwarts School of Witchcraft and Wizardry;

3. That our child will be leaving on September 12, 2019 and will stay in
the said country for a period of 10 months;

4. That he will be under the care of Albus P. Dumbledore, Headmaster


of the Hogwarts School of Witchcraft and Wizardry;

5. That we cannot personally travel with our child due to our hectic
schedules at each of our current occupations;

6. That we are giving our full consent for our minor child to travel
abroad.

7. That we personally guarantee financial support of our child in the


course of his travel and stay outside of the Philippines and further
guarantee that he shall not in any manner become a burden to the
Philippine and the United Kingdom governments.

5. That we are executing this affidavit to attest the veracity of the


foregoing facts and for whatever legal purposes this may serve.
