Background & Significance Results: Transitions of Care (TOC) Model Discovery Report
• Big data analytics has the potential to transform how
consumers use technology to gain meaningful insights

Purpose
To demonstrate the utility of Tableau® Dashboard to
represent data driven recommendations for transitions of
care (TOC) to post-acute care (PAC) settings

Materials
The Advocate Cerner Collaboration team created the TOC
Model, which provides decision support for the post-acute
Displays average readmission risk and rate over time in order to allow users to visualize
transition of care by recommending one of five settings:
